Context
Stanford is the most visible reference in AI health coaching, so it is worth being precise about what its projects proved versus what they only suggested. This matters because it is easy to cite “Stanford does AI coaching” as if it settled the question. It didn’t, and the nuance is the useful part.
The concept
Three projects, three lessons.
GPTCoach (CHI 2025) is an onboarding agent that uses motivational interviewing, pulls wearable data and offers visualizations through tools. It proved that an LLM can follow non-prescriptive strategies and gather qualitative context in a real conversation, instead of firing back a routine. The evaluation was small and focused on onboarding.
Bloom, with its agent Beebo (CHI 2026), extended that into a multi-week app. In a randomized 4-week trial with 54 people, both groups increased activity and there was no significant difference between the LLM and the control. The LLM group did show favorable changes in beliefs, enjoyment and self-compassion, plus a safety benchmark.
MHC-Coach (npj, 2025) is a Llama-3 model tuned to the Transtheoretical model of stages of change. In an evaluation with 632 participants, 68% preferred its stage-matched messages over human ones. The team also shipped a Spanish adaptation grounded in behavioral science rather than literal translation.
In practice
Design for agency and stage, not for compliance. Inferring where the person is in their change process and adjusting the type of support is the transferable idea. Set expectations honestly: a good conversational agent can improve experience and beliefs well before it moves hard behavior, and that already has value. Cultural adaptation is behavioral, not linguistic. MHC-Coach-ES is the model for a Spanish-first ADAPTY: adapt the science of the message, not just the words.
My take
The 4-week horizon is a weakness that repeats across the whole field. Almost no trial runs longer. Any claim of sustained change needs longitudinal evidence. This connects upward to [[emerging-architecture-health-coaching]], which shows where these systems sit in the loop. For ADAPTY, the Spanish and everyday-adult angle is underserved, but underserved is not the same as demand proven. It has to be validated with real users.
Contexto
Tres proyectos, tres lecciones.
