Context
I started mapping this field after a Stanford CS547 talk on AI health coaching. One thing stood out. The strongest labs, Stanford, Google, MIT, Toronto, are not racing to build a better fitness chatbot. They are converging on a shared architecture, and the interesting part is a design principle rather than a model.
The concept
The loop that keeps reappearing has seven steps. First perceive: text, history, wearables, self-reports, context. Then infer the state: barriers, motivation, stage of change, fatigue, uncertainty. Then generate several possible strategies instead of one. Then select, using rules, behavioral theory, a ranker, bandits, reinforcement learning or preference judges. Then communicate in language that preserves the person’s autonomy. Then observe what happened: acceptance, change of state, activity, adherence. And finally update memory, policy or profile for next time.
The key move is the one the Stanford slide “LLMs can rank candidate actions” captures. You separate the decision policy from the generator. The model stops being only a text producer and becomes part of a policy: infer the state, propose options, rank them by safety, relevance, autonomy and feasibility, and then speak. Systems like PHIA push this further by separating language from calculation. The model plans and explains while the tools run the actual time-series analysis.
In practice
This shapes how I think about PhysioFlow and ADAPTY. Do not mix calculation and persuasion in one prompt. I separate the analytical agent, which is verifiable and uses tools, from the behavioral agent, which talks and frames. I make the selection transparent: candidate actions ranked by explicit criteria, safety first, then relevance, autonomy and feasibility. And I communicate without prescribing, exploring the barriers before suggesting. The honest evidence, like Stanford’s Bloom trial, is that LLM coaching improved beliefs, enjoyment and self-compassion more than it beat a control on activity. Manage expectations with that in mind.
My take
The gap in the literature is real. Many systems analyze data without knowing how to accompany. Others converse well but do not calculate reliably. The differentiated bet is doing both, for everyday adults, in Spanish. Next notes to write: [[stanford-gptcoach-bloom]] on the coaching evidence, and one on governed longitudinal memory, how to learn over months without sending the full history each turn. Watch the honesty bar: winning on preference or benchmark is not sustained behavior change.
Contexto
